Information you provide

When you submit an appointment request, the website sends the details you enter to DCMD by email. The website does not store the submission in its own customer database. Email systems may retain messages under their own security and retention practices.

What not to submit

Do not send Social Security numbers, account numbers, identification images, medical details or confidential document contents through the website form. DCMD needs only enough information to understand the appointment request.

Analytics and security

DCMD uses Google Analytics and Vercel Analytics to understand website traffic and actions such as call clicks, text clicks and appointment-form activity. These services may process information such as the page visited, browser or device type, approximate location and interaction time. Appointment-form contents are not intentionally sent to analytics providers.

Google explains how it uses information from sites that use its services in its business data and privacy notice. Anti-spam and rate-limiting tools may also process technical request data to protect the form.

Your privacy choices

You may call or email DCMD to ask about information you previously submitted. Browser privacy controls may limit analytics cookies or scripts. DCMD does not sell personal information submitted through this website.
